RHA Salem Homes of Florida provides residential services for adults with disabilities to live in a safe and healthy environment throughout Florida.

Our goal is for people to achieve their highest level of independence at home and in the community. Our teams of healthcare and direct care professionals work closely with each person and their family members to provide support for all aspects of their life.

Residential Services in Florida

In Florida, we provide residential services in our Intermediate Care Facilities. These small, six-bed group homes for adults with disabilities are typical homes in the community. The people who live there are encouraged to pursue their personal goals and interests by engaging in community activities, shopping, dining out, going to concerts, and attending church or religious services of their choice.

  • 24/7 supervision
  • Transportation to appointments
  • Day program services
  • Work and recreation activities
  • Family-style nutritious meals with the oversight of a licensed dietician
  • Medication management
  • Occupational Therapy (OT)
  • Physical Therapy (PT)
  • Speech Therapy (ST)
  • Behavioral Health Services

Note: All services provided are included in the daily rate.

How Service Needs are Determined

An interdisciplinary team works with each person to identify and determine each person’s unique goals and supports. That team then designs appropriate services and specialized programs to provide positive outcomes for them.

If You Are Interested in Intermediate Care Facility (ICF) Homes in Florida:

Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IDD) is an optional Medicaid benefit that enables states to provide comprehensive and individualized health care and rehabilitation services to individuals to promote their functional placement authorization within 90 days. Please reach out to inquire about placement with us.

In Florida, all Medicaid recipients must initiate the need for services through the Florida Agency for Persons with Disabilities (APD).

Right to Placement Authorization *

The Agency for Persons with Disabilities is required to ensure that all individuals requesting ICD/IDD placement are authorized for admission to an ICF/IDD within 90 days. Individuals who meet ICF/IDD eligibility criteria have the right to be placed in an ICF/IDD while awaiting approval to receive services under the State Home and Community-Based Services Waiver.

What are the Eligibility Criteria for Placement?

An individual must be diagnosed as having an intellectual disability, disorder, or syndrome attributable to cerebral palsy, autism, spina bifida, or Prader-Will syndrome that requires 24-hour supervision and coordination, as well as the integration of health or rehabilitative services.
