Our large medical centers for people with disabilities provide the most intensive residential service available to individuals with intellectual/developmental disabilities. They are funded by Medicaid to serve individuals with intensive medical or behavioral needs who need more than a typical group home offers, but less than a skilled nursing level of care.
Bear Creek
La Grange, NC | Lenoir County
Bear Creek is home to 113 people and features 24-hour nursing and respiratory support. An on-campus school provides educational opportunities year-round. Care teams, which can include nurses, direct care staff, qualified professionals, respiratory and occupational therapists, physicians, and other specialists as needed, work closely with family members and guardians to create and implement a care plan that meets the unique needs of each person. Bear Creek is located in Lenoir County near LaGrange, NC.
Clear Creek
Charlotte, NC | Mecklenburg County
Clear Creek is home to 120 people with intellectual and developmental disabilities who receive 24-hour supervision and support services. The on-campus Hunter McNelly School provides educational opportunities year-round. Care teams, which can include nurses, direct care staff, qualified professionals, respiratory and occupational therapists, physicians, and other specialists as needed, work closely with family members and guardians to create and implement a care plan that meets the unique needs of each person. Clear Creek is located just outside of the Charlotte, NC metro area.
River Bend
New Bern, NC | Craven County
River Bend is home to 125 people with intellectual and developmental disabilities who receive 24-hour supports. The well-trained and professional staff provides a warm and nurturing atmosphere to all ages. Care teams, which can include nurses, direct care staff, qualified professionals, respiratory and occupational therapists, physicians, and other specialists as needed, work closely with family members and guardians to create and implement a care plan that meets the unique needs of each person. River Bend is located in the township of River Bend in New Bern, NC.
Tar River
Greenville, NC | Pitt County
Tar River provides compassionate care for children with the most challenging medical conditions. The state-of-the-art 30-bed facility offers nursing and daily care for birth-related complications, premature births, congenital defects, brain injuries and traumas. Tar River is located in Greenville, NC.
Walnut Creek
Goldsboro, NC | Wayne County
Walnut Creek is home to 37 people with intellectual and developmental disabilities who receive 24-hour supports. Educational curriculums are provided along with round-the-clock nursing. Care teams, which can include nurses, direct care staff, qualified professionals, respiratory and occupational therapists, physicians, and other specialists as needed, work closely with family members and guardians to create and implement a care plan that meets the unique needs of each person. Walnut Creek is located in Goldsboro, NC.
